The press conference to initiate the works on the building of the infrastructure for psychiatric rehabilitation by the deadline occurred on 20 June 2016. Representatives of Psychiatric Hospital Horní Beřkovice, aid organizations Fokus Praha, Labe and Liberec and the UJEP University took part in the discussion. The topic of the discussion was the specific activities of the Case Management team regarding psychiatric rehabilitation and publication of the results of the work with the target group. Present were representatives of construction company STAMO spol. s r.o. from Děčín, who explained the progress of the works. The program of the press conference included a tour of the site.

Project publicity – billboards at the construction site
18 May 2016
Within the publicity plan, billboards according to the approved manual were prepared and placed in both the B and O buildings at the commencement of construction works. Both billboards are placed on separate carriers in front of the building so that they are able to reach users on the local roads.

On 16 May 2016, investor Psychiatric Hospital Horní Beřkovice transferred the construction site to contractor STAMO, spol. s r.o.

Contract on Implementation of the Construction

During the course of the appeals period, no one submitted an appeal against the decision of the contracting authority on the best bid from STAMO, spol. s r.o., Company Id No: 43222323, with the winning price of 17,424,924 CZK, not including VAT. The Contract was signed on 10 May 2016.

Case Management team, the 1st stage of the project

The Case Management team met regularly according to the set time schedule from 15 April 2015 to April 2016.
As part of the Partnership Contract that was concluded, Psychiatric Hospital HoB works with domestic aid organizations Fokus Labe, Fokus Liberec and Fokus Praha. A time schedule of mutual cooperation within the Case Management team was prepared and approved. The basis of cooperation is the defining of the target group of patients. Regular meetings were extended into work with clients directly at the ward for the long-term hospitalized. The Fokus aid organizations are already involved in work with the target group in the existing premises. In addition, procedures were proposed for the creation of a common methodology for psychiatric rehabilitation. Joint work is based on the application of procedures from other Fokus worksites, and on identifying the needs of clients and their competencies. Within the case management activities, all of the decisive worksites of aid organizations in Ústí n. Labem, Mělník and Liberec were visited. Physiotherapy and occupational therapy were incorporated via a new project partner, the University of J. E. Purkyně in Ústí nad Labem, Department of Physiotherapy and Occupational Therapy, into the existing case management system. Comprehensive rehabilitation plans and the manner in which to use the CANN method were prepared. Fokus prepared a program for involving people with experience with mental illness in the rehabilitation process. Their role consists of feedback, which facilitates positive experience with the process of healing / recovery / to psychiatric patients who are currently hospitalized at psychiatric hospital HoB. In total, there have been over forty contacts, communications and managed motivational interviews with patients of the target group. The program activated 7 clients who left the hospital and went to sheltered housing.

SW HIPPO for psychiatric rehabilitation

The SW Hippo program for psychiatric rehabilitation was activated in the internal system of the Horní Beřkovice Psychiatric Hospital in January 2016. The program was submitted for pilot use on 21 March 2016. Doctors and health professionals use the program for diagnosing and recording activities with the client. In accordance with the Act on the Protection of Personal Data, the relevant data is transferred to employees of Fokus partner organizations.

Approval of the new implementation deadline
16 March 2016
A legal act approving the change was issued on 16 March 2016 by the grant provider. PN Horní Beřkovice (The Horní Beřkovice Psychiatric Hospital) can sign an agreement on implementation of the construction with the winner of the public contract.

Selection of a supplier VZ0008878
15 March 2016
New public tender VZ0008878 was announced on 31 December 2015. In total, 16 additional pieces of information were issued and opening of envelopes was extended until March. In total, 43 questions were submitted, 34 of which related to the construction and budget. A total of 22 construction companies submitted bids. Of the eleven correct bids, the winner was STAMO spol. s r.o. from Děčín. In view of the significant interest of construction companies in carrying out the construction contract, the bids of the competitors were comparable. The planned budget set by the designer from the prices usual for ÚRS (the Institute of Rationalization in Civil Engineering) items was decreased by 28.03% by the winning bid. The difference between the price of the budget and winner’s bid is 6,805,481 CZK.

Extension of the implementation deadline
23 January 2016
The reconstruction activity does not take place in accordance with the implementation plan. In view of the errors in the contractual documentation – material errors in the project – the request for changing the extension of the deadline was submitted as a Request for a Change on 23 January 2016. The extension of the deadline to 30 April 2017 will create enough room to carry out the construction.

3 January 2016
A public tender for choosing a supplier was announced on 31 December 2015. The final deadline for implementation is set for 30 April 2016. After consulting with the grant provider, the project team will submit a request to extend the implementation deadline.

Selection of a supplier VZ006903
26 November 2015
The tender for the supplier of the construction part of the project was initiated on 6 October 2015 as VZ006903 on the profile of the contracting authority. Via its decision on 26 November 2015, the contracting authority cancelled the tender on the basis of persistent shortcomings in the project documentation. Competitors sent a total of 83 questions, 78 of which concerned the construction.

Hippo presents a new program
19 November 2015
The Case Management team communicated with the creator of the program and defined the requirements for the SW architecture, which would be compatible with the existing systems. The preparation of the program progressed, and it was therefore decided to invite the creator to the group for planning psychiatric rehabilitation on 19 November 2015. The presentation of the program and its functional parts was to prove that the requirements for the functionality of the system for partners of the project were also fulfilled. Part of the presentation of the system and discussion of the functionality and purposefulness of the proposed solutions.

6 October 2015
A public tender for choosing a supplier was announced on 6 October 2015. The final deadline for implementation is set for 30 April 2016. The qualification requirements of the tender documentation are determined in such a way that a wide spectrum of quality suppliers can take part in the tender.

Characteristics of the Psychiatric Hospital of Horní Beřkovice

The state healthcare institution of Psychiatric Hospital (PH) Horní Beřkovice focuses on a complex therapy of psychiatric diseases. The catchment area of the hospital comprises about 1.5 million inhabitants, covering predominantly the Ústí nad Labem Region, but also a part of the Liberec Region and Mělník and Kladno Districts.

Patient numbers targeted by the project (treatment of mental illness) reach the total of 220 persons with a psychiatric diagnosis.

The plan has been designed to achieve crucial elimination of a weak point in health and social care of patients with the psychiatric diagnosis of a mental illness. PH Horní Beřkovice provides system services focused on the development of skills enabling the clients to find their role in the society.

The system operation of PH Horní Beřkovice is connected to the results of the three-year Follow-Up After-Treatment Care Programme implemented in cooperation with the non-profit sector. Patient needs were successfully identified, and a complex system of integrated rehabilitation was set within the existing conditions of the infrastructure, which is available for the needs of the patients and for implementation of social rehabilitation elements.

The Psychiatric Hospital infrastructure is continuously completed and reconstructed based on a long-term concept, so as to comply with requirements of efficient treatment of long-term ill patients. New places of work have been created for social rehabilitation of long-term hospitalized patients with a serious mental illness (SMI). In this group, the social rehabilitation programme is fulfilled to the extent of 50% of the planned activities. The places of work utilized include a Multifunctional Culture Centre, the Garden Work Centre, and the Joinery Workshop. As regards the art therapy, a technically obsolete ceramic workshop with insufficient space is used. Establishment of an Internet Café proved to be a strong activization element, used for familiarization with communication technologies and with the related programme of the JOB centre – working at the café as a part of therapeutic activities. Meeting rooms for individual work with the clients are limited in space. The training is theoretic, and it is continued within after-care premises of our partners at helping organizations when the clients are discharged.

• F10-F19 Mental and behavioural disorders due to psychoactive substance use
• F20-F29 Schizophrenia, schizotypal, delusional, and other non-mood psychotic disorders: 39% (men 30%; women 10%)
• F30-F39 Mood (affective) disorders: 4% (men 3%; women 1%)
• F40-F48 Neurotic, stress-related and somatoform disorders: 2.9% (men 1.4; women 1.5%)
• F60-F69 Disorders of adult personality and behaviour: 9.5% (men 8.5%; women 1%)

Complex rehabilitation centre of PH Horní Beřkovice

PRESS INFORMATION

The psychiatric hospital of Horní Beřkovice (PH Horní Beřkovice) has obtained a financial grant of CZK 20.819.125,-- CZK to build the Complex Rehabilitation Centre of PH Horní Beřkovice. The funds will be provided from the Norway Grants Financial Mechanism administered in the Czech Republic by the Ministry of Finance of the Czech Republic. Grant No. NF-CZ11-OV-2-028-2015 was awarded based on a well-prepared plan to establish the methods of integrated psychiatric rehabilitation.

In 2014, the Ministry of Health of the Czech Republic as an intermediary authority issued a call for grant applications. The Norway Grants prepared a number of subjects for the Czech Republic, which were not supported through European Union grants. The programme C11, Public Health Initiatives, was focused on the improvement of psychiatric care services. The grant was designed to improve healthcare in the fields that are underfinanced in the long term and whose development is below the level of advanced EU and EEA countries. Together with the project partners, i.e. organizations engaged in the development of non-state social services, PH Horní Beřkovice has prepared a complex rehabilitation programme that includes both hospital treatment as well as social after-care. The following FOKUS association organizations have become partners of the project: Fokus Labe, Fokus Liberec, and Fokus Praha-Mělník.

Project objectives are:

• to enhance the quality and efficiency of therapeutic processes in order to shorten the duration of hospitalization of psychiatric patients;
• to increase the effectiveness of the therapeutic process, destigmatize and transform the psychiatric care.

The project plan of PH Horní Beřkovice was designed as an elimination of a weak point of health and social care of persons with disabilities. The Complex Rehabilitation Centre of PH Horní Beřkovice will be designed particularly for patients with a serious mental illness (SMI). These are clients hospitalized in the long term who have a diagnosed psychiatric mental illness such as mental disorders due to psychoactive substance use, schizophrenia, affective disorders, neurotic, stress-related and somatoform disorders and personality and behavioural disorders.

The aim of psychiatric rehabilitation in the conditions of PH Horní Beřkovice is to help persons with psychiatric conditions to enhance their ability of functioning in order to be successful and contented in the environment they have chosen to live in, with the minimum possible level of permanent professional support. A successful purpose of complex rehabilitation is to enable patients with a serious mental illness to become better integrated in the society outside the psychiatric hospital.

Creation of conditions for the development and implementation of a complex rehabilitation care programme is a prerequisite for initiating the integrated rehabilitation programme. The psychiatric rehabilitation programme is currently restricted in terms of space given that PH Horní Beřkovice does not have an available technical background. A number of activities are implemented to a limited extent.

As envisaged by the project plan, the Complex Rehabilitation Centre of PH Horní Beřkovice will be designed as an independent department with related places of work, operated on the community principle. Both training sites will be newly built and their construction will be supported by funds from the Norway Grants.

Investment in new infrastructure for complex rehabilitation

The Training centre of manual skills will be created through the reconstruction of Object O, a former coal storage area, converting it into a two-floor rehabilitation site.

Newly created will be ergotherapeutic workshops and places of work:

1. Gym
2. Workshops comprising 8 independent areas

Workshop A – ceramic and basket-making techniques
Workshop B – processing of polymeric materials, wax and soap-making workshop
Workshop D – production of paper, jewellery, graphics and varnishing
Workshop C – Processing of textiles, craft techniques, work with silk

SO2 – Coal storage; Training centre of manual skills and art therapy
The training centre of social skills will be created through the reconstruction of the second floor above ground of Object B, a former monastery within the psychiatric hospital premises. New places of work are connected to rooms used in after-care for interactive contacts between clients and non-profit after-care organizations. Newly created will be the following places of work:

1. Training laundry room
2. Training kitchenette
3. Therapeutic area for individual consultations, meeting room, 5 places of work
4. Area for community meetings – therapy
5. Room for PC training of cognitive functions

Rehabilitation Planning Group
In the process of preparing the infrastructure, a structured programme was also prepared in cooperation with regional FOKUS association project partners for the needs of the clients; the programme is focused on occupational diagnosis, training of skills, and therapeutic training aimed at overcoming crisis situations.

In April 2014, PH Horní Beřkovice and Fokus Labe, Fokus Liberec and Fokus Praha-Mělník established the Rehabilitation Planning Group; this group has already been preparing the methods for joint work so that an integrated rehabilitation programme can be initiated when the construction is finished in April 2016.

The concept of building the rehabilitation centre is based on personal experience of physicians, psychologists and healthcare providers with these patients at the hospital, and on limited possibilities of their integration outside the hospital in the catchment area of the Ústí nad Labem, Central Bohemian, and Liberec Regions. The project actively responds to new findings in the fields of psychotherapy and after-care provided by non-state organizations in the area of social integration.
The project is intended to help target patient groups to access good-quality healthcare and to become reintegrated in civil life, and to improve the prevention of subsequent health and healthcare related complications.

The programme of the Complex Rehabilitation Centre is divided in 4 basic phases, which are chronological, but which also alternate in cycles:

1. Diagnostic and motivation phase
2. Planning phase
3. Rehabilitation phase
4. Evaluation phase

Activities are focused particularly on three consecutive phases of the recovery process, stabilization, and especially reorientation and reintegration. The number of rehabilitations is anticipated according to the needs of individual clients based on checking the progress achieved by them. The complex rehabilitation programme lasts for 3 to 12 months according to the determined progress of the clients. Any group has 3 to 8 clients. Also scheduled are client visits at specialized places of work of Fokus Labe, Fokus Liberec and Fokus Praha-Mělník partners, so that after the hospitalization, the clients be able to engage in subsequent social integration.

Principal therapeutic goals of the integrated rehabilitation method:

• to minimize or stabilize the symptoms of the disease
• to prevent re-hospitalization
• to support the ability of independent functioning
• to support the ability to work and study
