Transformation of psychiatry and the rehabilitation departments

In May 2018 a team for transformation of the hospital was established as part of the transformation process, with participation of PNHoB and community service representatives. First meetings were held incl. one with participation of representatives of the regional and national authorities.

The team builds on a close cooperation between the hospital and community services, which was established within the scope of the project supported by the Norway funds. This facilitates communication and cooperation incl. decision-making on the level of management bodies of cooperating subjects.

In the upcoming individual work with difficult patients we will greatly benefit from the established quality infrastructure as well as the cooperation platform of the hospital and community services.


Successful accomplishment of the first year of project sustainability period

On April 30 we successfully accomplished the first year of sustainability of the project „Centre of Complex Rehabilitation“ supported by the Norway funds. The project indicator of 80 patients undergoing the rehabilitation program was fulfilled. We have further developed the rehabilitation methodology incl. educating the staff in the CARe approach. We also have further broadened and intensified the cooperation with community services. The whole multidisciplinary team deserves appreciation: the 2A and 3A departments, the Centre for psychosocial rehabilitation and the Centre for occupational therapy


Seminar „Centre of complex rehabilitation from the perspective of non-medical staff“

On April 25 a seminar was organized in the Multicentre of PNHoB focused specifically on the role of non-medical workers in the rehabilitation of psychiatric patients. The idea came from the initiative of nurses in the rehabilitation departments and was accessible for all PNHoB personnel. Presentation were given by

  • Bc. Miroslava Brůžová (head nurse 3A): CKR in PNHoB and the CARe approach,
  • Pavla Danilovová (head social nurse) : Cooperation with Fokus units and social therapy activities,
  • Bc. Dagmar Šťastná (head nurse 2A): Case study of patient of dep. 2A,
  • Bc. Květa Myslivcová (head occupational therapist): Occupational therapy intervention.

Visit from PN Jihlava in PNHoB and Platform CARe meeting

The team of Psychiatric Hospital Jihlava visited the rehabilitation departments of PNHoB on April 9. Our colleagues were interested in the rehabilitation methodology, but also in technical aspects such as using the P module in the information system Hippo. We shared best practices and discussed the systemic obstacles we have to overcome in implementing efficient rehabilitation.

We were invited to continue the discussion in the Platform CARe meeting in Jihlava, where Mgr. Procházka took part. Here we suggested some topics and work-format for the upcoming Conference of social psychiatry.


From patient to client: The way of patient from PNHoB to community services

On March 28 the Multicentre of PNHoB hosted a meeting of the wider teams of rehabilitation departments 2A and 3A and all the workers of the cooperating community services. All personnel of PNHoB was allowed to participate in the formal presentations. The objective was to

  • Inform all medical and non-medical hospital workers about community services
  • Unify cooperation with all community service organizations
  • Expand the cooperation by including new community services
  • Inform the 2A and 3A dep. nurses about types of services and their operation in detail
  • Meet with key workers in person

The cooperation framework was presented by Mgr. Procházka. Presentations followed of Fokus Labe, Fokus Liberec a Česká Lípa, Fokus Mělník, Fokus Dolní Povltaví, Baobab Kladno, Charita Česká Kamenice. The meeting contributed to information of PNHoB staff about cooperation with community services and also to a closer contact between the key workers.


Complex education of patients by key nurses

We have elaborated the education of patients in the rehabilitation departments according to the structure of the CARe personal profile. It now contains 11 information blocks: housing, work, leisure time, education, health, self-care, relationships, safety, my disease, crisis plan, life with my disease. The education is driven by key nurses in small patient groups. We use own materials and the CESTA package.

Patients in the rehab departments often lack basic information, knowledge and skills necessary in daily life. Their situation is even more complicated than that of other people, because they have to understand their medication and take it regularly, they may feel stigmatized etc.

They sometimes realize this handicap, which may negatively influence their self-confidence and lead to dependence on others. This is the focus of education: to improve basic skills necessary in everyday life (sometimes with support of community services) and to foster cooperation with farmacological treatment. The objective is to

  • Build competencies of patients in areas related to quality of life.
  • Engage the patient actively into treatment. To help him understand the disease and cope with it. To prevent stigmatization.
  • Build a therapeutic relationship between the patient and the nurse.
  • Increase the knowledge of the key nurse about the case and story of the patient.

Successful accopmlishment of the CARe training

February 27 was the last training day of the accredited CARe training of personnel in the rehabilitation departments of PNHoB. The staff of departments 2A and 3A have gained their accomplishments.

The course contained the theoretical introduction to basic concepts of the CARe approach like recovery, empowerment and presence. It contributed to the attitude change of the workers in the direction of positive approach to patients based on strengths in the spirit of the psychiatric reform.

The participants also appreciated the practical dimension of the training and feedback from the trainer Mgr. Jana Pluhaříková-Pomajzlová. They could practice using the specific CARe tools like personal profile, individual plan, crisis plan, map of social network etc.


Seminar on psychiatric rehabilitation for doctors

On the 8th of February we organized a seminar for all medical doctors in PNHoB focused on psychiatric rehabilitation. Presentation was given by MUDr. Hadjipapanicolaou and Mgr. Procházka.

The lecture mentioned a brief history of psychiatric rehabilitation and introduced the main approaches and schools: Los Angeles school, Boston school, British school and Dutch school. We discussed the concept of recovery and phases of the rehabilitation process. We hope to have contributed to the change in thinking of the medical community in accord with the ongoing psychiatric reform.


Case management with patients in cooperation with Fokus

Starting in January 2018 we take an important step forward in case management of patients during his reintegration after dismissal. The case workers of community organizations will now be in closer contact with rehabilitation department staff.

The community workers of Fokus will now regularly visit the rehabilitation department and participate in case management and planning team sessions. This has multiple benefits:

a) The contact will be more frequent: once every two weeks minimum

b) There will be more time to communicate about and with individual patients

c) There will be an opportunity to bring in the patient to a joint session with PNHoB and Focus participating

d) The department case workers can now communicate directly with Fokus case managers

e) The planning process will be more coordinated

The intensive information sharing will enable us to be more consistent and flexible in case management, esp. of those patients who do not share information about stated agreements. The case managers will also know each other personally and will be in close touch. There will be important information available from the department staff about daily functioning of the patient, a source hardly substituted by other workers.


Phases of the rehabilitation process

We have implemented a new system of rehabilitation process phasing in department 2A. It involves 3 phases of treatment in accord with the CARe method.

In the first, stabilization phase, the department staff focuses mainly on stabilizing the patient´s health condition. The physician plays the main role here, of course. She gradually optimizes the medication in order to minimize symptoms, but also the side-effects of treatment that might interfere with the rehabilitation process. The activities are therapeutic according to the wishes and capacity of the patient.

In the second, diagnostic and therapeutic phase, the focus is on thorough assessment of the needs and wishes of the patient, and administering psychiatric scales and psychological tests. Patient is now able to participate in group and individual psychotherapy. The activities are aimed at activation. The psychologist is now the main driving force of the process besides the case worker.

In the third, reintegration phase, the patient is gradually prepared for dismissal from the hospital. Activities now focus on developing self-discipline and self-reliance, but also specific skills like washing and cooking. The patient is now fully acquainted with the department and is expected to participate actively in community roles. Important role is now played by social workers in cooperation with community services, f. ex. the Fokus organization.


Medication education using video

Indicated patients in the rehabilitation departments 2A and 3A are now being offered a series of 5 group sessions focused on improving their knowledge of drug treatment and improving compliance.

The session are facilitated by a medical doctor and psychologist in an interactive format using short video clips of patient – doctor interactions. The patients are encouraged to relate to the information presented and bring in their individual concerns. The topics focus on

  • discerning symptoms of disease and building insight,
  • the effect of drugs and understanding of the necessity of continual treatment,
  • relief from side-effects,
  • advantages of depot medication,
  • open dialog with the medical doctor,
  • taking medication and out-patient treatment,
  • other related topics.

Implementing supervision of CARe approach in rehabilitation depatrments

The first methodic supervision took place in the rehab departments 2A and 3A. Facilitated by the Platform CARe trainer Mgr. Jan Pluhaříková-Pomajzlová the session focused on creating the initial contract. The teams agreed to undergo supervision on regular basis and the hospital management agreed to provide funding.

The supervision objective is:

  • to provide methodic support in implementing rehabilitation program in PNHoB,
  • to help improve cooperation with cooperating organizations and their employees,
  • to gradually improve rehabilitation services for PNHoB patients.

Successful project audit

On October 13 2017 an audit of the Norwegian Funds EHP funded project was carried out. It focused on the system of financial management of the project and also on the effect of invested funds.
The beneficiary presented the project documentation to the audit body and briefed it about project outcomes. An audit of accounting was carried out subsequently and eligibility of claimed resources was checked.

No problems were found in the audit. Therefore no control findings were formulated and no corrective measures imposed. The audit has shown, that resources are used in accord with the Decision to provide the grant.


Pilot phase of the patient´s personal schedule

Implementing the “vitamin” regime of the Center for Complex Rehabilitation, we have decided to launch an organizationally challenging change – a patient´s personal schedule. Every patient now receives a weekly schedule of rehabilitation activities, which he is obliged to follow. The activities are indicated according to his needs and preferences; the schedule is updated when it makes sense with regard to rehabilitation.

The organizational challenge was in the necessity to change the former daily planning process, which is not advantageous for rehabilitation. Being sent to activities on daily basis the patients were falling into a passive mode, responsibility for the daily routine laid on the department personnel.

The new system of individual scheduling transfers the responsibility for organizing their time on patients themselves. This model is much closer to reality outside the hospital, where daily planning is based on one´s own initiative, deciding and planning, how one will spend his time. This is particularly important for patients with disability pensions, who´s chances of work involvement are limited. This will hopefully further approximate the department operation to the reality of everyday life, facilitating the patients´ transition on their way out.

The community principles of the hospital department are an important part of successful rehabilitation. Patients assume various roles thereby learning to assume responsibility for a certain area of activity. This helps to activate them and build their competencies, their self-esteem and assertiveness grows. This is why we slowly reinforce community elements of the department regime.

As of today we introduce 3 new community roles and significantly enrich the existing roles in the department. There is the chairman and deputy, and newly the operations officer, sports organizer and culture program manager. The responsibilities and duties are fairly extensive, f. ex. the sports organizer manages several sports activities during the week incl. regular exercises, organizes a trip; the culture program manager plans 5 cultural events in the department and in the Multicenter of PNHoB.

We expect patients to succeed in these roles and we are eager to see the positive rehabilitation effects. In the positive case we plan to further develop community roles and principles in the project.


We welcome new occupational therapists

Implementing the project of Complex Rehabilitation, we struggle with low number of available occupational therapists. Our advertising incl. this add remains unsuccessful.

We are trying to assess the situation with the help of the Faculty of Medical Studies of the University of Jan Evagelista Purkyně Ústí nad Labem. It follows from the discussion with PhDr. Hana Kynštová, Ph. D. from the Occupational Therapy Department, that the problem lies in a low number of available occupational therapist on the market combined with low attractiveness of the remote work location of Horní Beřkovice. Nevertheless she is trying to help us by arranging “directed internships” of occupational therapy students in PNHoB. The students would help with the operation of workshops and at the same time enjoy an interesting practice in a modern department.

In this context we can happily announce, that during August 3 new occupational therapist assume their jobs in the Center for Training of Manual Skills. This raises the capacity of the workshops significantly and utilization of the new premises improves. Patients can also be worked with on a more individual basis. Welcome, colleagues!


FOKUS community workers now “hold office” in our hospital

In the reconstructed building of the Training Center for Psychosocial Skills, two rooms are now fully at the disposal of FOKUS community workers. They can now work with patients in representative and fully equipped consulting rooms, and not in limited circumstances of the hospital departments as before. They enjoy silence and privacy, and patients appreciate it as well.

Furthermore, these rooms are accessible at any time, so they can arrange meetings with patients directly with nurses in the hospital departments. To avoid overbookings, there is an online reservation system available, for which we are grateful to our IT department.

We wanted the hospital to be as “permeable” to the workers of FOKUS as possible. They currently have de facto detached offices in the newly reconstructed premises of the psychiatric hospital, and they can work extensively with patients during their hospitalization. This is vital for establishing a good therapeutic relationship as a basis for cooperation with the patient after discharge. We wish FOKUS a pleasant stay on our premises!


We improve rehabilitation planning with patients

New system of rehabilitation planning was launched in rehabilitation departments of the Psychiatric Hospital Horní Beřkovice. Monthly planning sessions of the whole multidisciplinary team take place for each patient participating in the project: the session is conducted by the case manager with the doctor, psychologist, occupational therapist and social worker participating.

The case manager, knowing best the patient´s history and situation, evaluates his progress in achieving rehabilitation goals. The whole team strives to reinforce the patient´s motivation, confirm his success and appreciate the commitments he had fulfilled. The team than explores the patient´s wishes and vision for the future, esp. after discharge from the hospital. In accordance with this the team than sets rehabilitation goals for the coming period in cooperation with the patient.

The objective of these meetings is to coordinate the strategy of the whole team, put it in accord with the patient´s wishes and logic of his personal story, and to motivate the patient to assume responsibility for further rehabilitation activities. In doing so he will often have to tackle many difficulties, face challenging social situations and change habits, which is why reinforcing motivation is so crucial for rehabilitation success.


CARe rehabilitation training in the Center for Complex Rehabilitation

On the 5th of June a long term training was launched in the Psychiatric Hospital Horní Beřkovice. The medical staff of Center for Complex Rehabilitation undergo a series of seminars in the CARe rehabilitation methodology. The training conducted in cooperation with the CARe Platform, involves five full-day seminars over a span of six months including intervisions of participants with trainers in the meantime. The doctors, nurses and the psychologist take part.

The CARe methodology (“Comprehensive Approach to Rehabilitation”) if used for over 25 years by professionals in the Netherlands and other countries, and in recent years it grew roots in our country as well. Implementation of CARe principles in the rehabilitation of patients will enable a smooth transition of patients leaving the hospital into community care and will facilitate communication between medical staff and community service workers of FOKUS and other organizations.

The CARe approach is a complex rehabilitation methodology and covers many aspects of the rehabilitation process, some of which are applicable in the hospital conditions: case management of patients, assessment of their wishes and personal profiles, planning of the rehabilitation process, “vitamin” regime of the hospital department etc.

Doctors in the Psychiatric Hospital Horní Beřkovice are trying hard to help patients live a normal life. Especially to those, who struggle with a severe disease in the long term. Thanks to extensive reconstructions in the hospital premises they now have new facilities at their disposal.

“The fact that we obtained new premises has helped us a great deal. Firstly the capacity has grown, but importantly as well, patients now spend their time in premises of a considerably higher standard,” explains psychologist and program manager Marek Procházka.

The objective of the Complex Rehabilitation Project is to help the patients recover well enough to be able to live a quality life despite their disability, have satisfying relationships and fulfill their needs and wishes. “We use all kinds of creative techniques. You can weave, sew, and knit,” lists the Occupational Therapy Head Květa Myslivcová and adds: “It brought us expertise – occupational therapy is no longer perceived as a mere leisure time activity. It proves its usefulness in developing autonomy of patients and in adaptation after leaving the hospital.”

The new premises for Complex Rehabilitation of patients have been built for 21 million CZK thanks to funding from the Norway Grants.


New gym is popular with patients

In the newly reconstructed coal storage building of the Horní Beřkovice Psychiatric Hospital a gym has been opened. Patients can now engage in therapeutic exercise under supervision of occupational therapists. They can use modern gym equipment: stepper, orbitrack, exercise bike, overballs, gymballs, wall bars, flexibars, flexi-pulls, massage balls and mats.

Thanks to the project of complex rehabilitation a new important option was added to enhance the physical rehabilitation of patients. Formerly patients could use the hospital sports and exercise ground, now the possibilities grew wider and are available during winter as well.

Patients are excited by the exercise lessons and a positive effect is apparent on some of them. Regular physical activity is an important part of the rehabilitation process. It assesses the problem of low physical activity of patients in hospital departments and it brings a new regular element into the weekly routine. It is an opportunity for patients to experiment with new hobbies that they can transfer into daily life after discharge from the hospital – thereby solving an often uneasy challenge of how to spend free time in a satisfying way.

Newly reconstructed premises of a former coal storage house in the Psychiatric Hospital Horní Beřkovice were opened in May. The coal storage building has been transformed into a Center for Manual Skills and handed over to hospital occupational therapists. Former workshops for ceramics, handmade paper and textile were moved here and new equipment for wax and silk processing was launched into operation.

For patients undergoing rehabilitation who often spend a long time in the hospital this is a welcomed upgrade of occupational therapy. New activities are attractive for patients who did not prefer or enjoy the previously available options. It is easier now for occupational therapists to engage these patients, at the same time having more tools to rehabilitate various functions.

New representative premises and a fine high standard work environment – this also has an important effect on self-esteem of patients and their subjective experience of occupational therapy.

The hospital integrates patients into society

The Psychiatric Hospital in Horní Beřkovice, Litoměřice Region, opens new buildings for rehabilitation of psychiatric patients. These are part of the new Center for Complex Rehabilitation which helps to re-integrate patients back into regular social life. Total cost of the project was almost 33 million CZK.

The rehabilitation departments should facilitate the transition of patients into normal day-to-day life. In one of the buildings there is a training kitchen and laundry. Patients can practice daily chores here like cooking, washing and ironing. Second building features a Center of Manual Skills where patients make ceramic pots, textile products and exercise in a gym.

According to psychologist of the Center for Complex Rehabilitation Marek Procházka patients often fail when they leave medical care and re-enter regular life, returning quickly into the hospital. The project target group are patients with Severe Mental Illness and patients in long-term hospital care. After discharge patients seek help of social and community centers FOKUS, partner of the hospital. “The Center is meant to be a bridge between the hospital and community social services,” says Procházka.

The press conference to initiate the works on the building of the infrastructure for psychiatric rehabilitation by the deadline occurred on 20 June 2016. Representatives of Psychiatric Hospital Horní Beřkovice, aid organizations Fokus Praha, Labe and Liberec and the UJEP University took part in the discussion. The topic of the discussion was the specific activities of the Case Management team regarding psychiatric rehabilitation and publication of the results of the work with the target group. Present were representatives of construction company STAMO spol. s r.o. from Děčín, who explained the progress of the works. The program of the press conference included a tour of the site.

Project publicity – billboards at the construction site

Within the publicity plan, billboards according to the approved manual were prepared and placed in both the B and O buildings at the commencement of construction works. Both billboards are placed on separate carriers in front of the building so that they are able to reach users on the local roads.

Case Management team, the 1st stage of the project

The Case Management team met regularly according to the set time schedule from 15 April 2015 to April 2016.
As part of the Partnership Contract that was concluded, Psychiatric Hospital HoB works with domestic aid organizations Fokus Labe, Fokus Liberec and Fokus Praha. A time schedule of mutual cooperation within the Case Management team was prepared and approved. The basis of cooperation is the defining of the target group of patients. Regular meetings were extended into work with clients directly at the ward for the long-term hospitalized. The Fokus aid organizations are already involved in work with the target group in the existing premises. In addition, procedures were proposed for the creation of a common methodology for psychiatric rehabilitation. Joint work is based on the application of procedures from other Fokus worksites, and on identifying the needs of clients and their competencies. Within the case management activities, all of the decisive worksites of aid organizations in Ústí n. Labem, Mělník and Liberec were visited. Physiotherapy and occupational therapy were incorporated via a new project partner, the University of J. E. Purkyně in Ústí nad Labem, Department of Physiotherapy and Occupational Therapy, into the existing case management system. Comprehensive rehabilitation plans and the manner in which to use the CANN method were prepared. Fokus prepared a program for involving people with experience with mental illness in the rehabilitation process. Their role consists of feedback, which facilitates positive experience with the process of healing / recovery / to psychiatric patients who are currently hospitalized at psychiatric hospital HoB. In total, there have been over forty contacts, communications and managed motivational interviews with patients of the target group. The program activated 7 clients who left the hospital and went to sheltered housing.


SW HIPPO for psychiatric rehabilitation

The SW Hippo program for psychiatric rehabilitation was activated in the internal system of the Horní Beřkovice Psychiatric Hospital in January 2016. The program was submitted for pilot use on 21 March 2016. Doctors and health professionals use the program for diagnosing and recording activities with the client. In accordance with the Act on the Protection of Personal Data, the relevant data is transferred to employees of Fokus partner organizations.

Approval of the new implementation deadline
16 March 2016
A legal act approving the change was issued on 16 March 2016 by the grant provider. PN Horní Beřkovice (The Horní Beřkovice Psychiatric Hospital) can sign an agreement on implementation of the construction with the winner of the public contract.


Selection of a supplier VZ0008878
15 March 2016
New public tender VZ0008878 was announced on 31 December 2015. In total, 16 additional pieces of information were issued and opening of envelopes was extended until March. In total, 43 questions were submitted, 34 of which related to the construction and budget. A total of 22 construction companies submitted bids. Of the eleven correct bids, the winner was STAMO spol. s r.o. from Děčín. In view of the significant interest of construction companies in carrying out the construction contract, the bids of the competitors were comparable. The planned budget set by the designer from the prices usual for ÚRS (the Institute of Rationalization in Civil Engineering) items was decreased by 28.03% by the winning bid. The difference between the price of the budget and winner’s bid is 6,805,481 CZK.

In view of the errors in the contractual documentation – material errors in the project – the request for changing the extension of the deadline was submitted as a Request for a Change on 23 January 2016. The extension of the deadline to 30 April 2017 will create enough room to carry out the construction.

A public tender for choosing a supplier was announced on 31 December 2015. The final deadline for implementation is set for 30 April 2016. After consulting with the grant provider, the project team will submit a request to extend the implementation deadline.


Selection of a supplier VZ006903
26 November 2015
The tender for the supplier of the construction part of the project was initiated on 6 October 2015 as VZ006903 on the profile of the contracting authority. Via its decision on 26 November 2015, the contracting authority cancelled the tender on the basis of persistent shortcomings in the project documentation. Competitors sent a total of 83 questions, 78 of which concerned the construction.


Hippo presents a new program
19 November 2015
The Case Management team communicated with the creator of the program and defined the requirements for the SW architecture, which would be compatible with the existing systems. The preparation of the program progressed, and it was therefore decided to invite the creator to the group for planning psychiatric rehabilitation on 19 November 2015. The presentation of the program and its functional parts was to prove that the requirements for the functionality of the system for partners of the project were also fulfilled. Part of the presentation of the system and discussion of the functionality and purposefulness of the proposed solutions.
